MB Wealth offers two convenient ways for you to open an account with us and begin trading.
Click the first link to open your account online via a secured connection, no one will see your information since it is encrypted before it is sent. This method is the most efficient.
Click the second link in order to request an application in physical forms either by downloading a pdf format of the application and then printing it or having them send it directly to you via mail.
Whether you create your account electronically or by mail, it is required that you return a photocopy of a valid state or country issued I. D. You can find our fax number and address on the "contact us" page.

Account Opening Tips
Individual Accounts
The page required for individual accounts are: 3, 4, 13, 14, and 16
Joint Accounts
The pages required for a join account are: 3, 4, 13, 14 and 16 Please remember that there are two boxes that can be checked for joint accounts:
- Joint Tenants with rights of Survivorship (usually husband and wife)
- Joint tenants in Common (an example would be two friends opening an account together)
For joint accounts they need to put the % of ownership on the account - must equal 100% (most often 50/50)
Trust Accounts
Trust Accounts are filled out just like an individual account, however the customer needs to also sign and date page 34 in the account book which is the Personal Guarantee. We would also need them to send in a complete copy of their trust agreement. What we are looking for in the Trust Agreement is that the customer/s who filled out the paperwork are listed in the trust and that it states in agreement that they can trade futures, commodities etc. Page 34 is required for Trust paperwork.
Corporate or LLC Accounts
Corporate and LLC accounts are filled out like Individual accounts, with the following exceptions: the top of page 3 needs to be checked Corp or LLC and the account information will be under the name of the Corp or LLC. The customer info should be for the individual(s) from the Corporation or LLC that are filling out the paperwork. Pages 32-34 are needed. With Corp. or LLC accounts they need to send in a complete set of there articles of Inc. We are looking for the name of the person who filled out the PFG paperwork to be listed in the articles and to have authority for the company to open accounts. We also need to know what the objective of the company is, if it has anything to do with financial industry we will need a Corp. Own funds Letter.
Partnership Accounts
Partnership accounts are filled out just like a Corp or LLC accounts but we only need page 34 opposed to 32-34 as with the corp. and LLC accounts. We will need a copy of the Partnership Agreement and again we are looking to see that the person who filled out the account is listed in the partnership.
IRA Accounts
IRA Accounts are filled out the same as individual accounts, however the box that is checked at the top should be IRA/Pension. For all IRA accounts page 34 is required. IRA accounts are to be sent to the IRA Company first for approval. Once the IRA account is approved by the IRA Company they will send it to PFGBEST to open.
Foreign Accounts
On Foreign accounts the customer is required to send us a Government issued photo ID and the W8 Ben Form.
Foreign customers living in the U.S.
For customers who are not US Citizens but living and working in the US they should have a Social Security number and will need to fill out page 14 – a photo ID is also required.
